Phase 4

Phase IV is a voluntary phase. This Phase is designed to assist you and your child in making the transition from the structure of Drug Court and probation to mastering the learned skills and demonstrating positive independence. Your contact person would be the Drug Court Coordinator(s)

Goal 1: Aftercare Planning

Prior to program graduation, you and your child will meet with counselors, and any others identified to formulate a post-graduation case plan focusing on accomplishing identified goals, in conjunction with:

  • Encourage your child to schedule and participate in ongoing positive social activities and friendships
  • Encourage and assist your child in enrolling or continued involvement with employment or skill building program
  • Encourage and remain involved in your child's continued progress on educational goals; by regularly checking on attendance and meeting with teachers
  • Continue to model respectful communication for your child
  • Sustain your involvement and coordination of regular family activities, meal times, maintain your family rules, and expected behaviors
  • Be involved in your child's Relapse Prevention planning
  • Familiarize yourself with your child's identified support network, specific sponsor or support person(s) and/or group
  • Encourage and participate in your child's continued substance abuse services and/or 12-Step, SMART Recovery or other community based support group
  • Participate and assist your child in their continued involvement and utilization of Drug Court support
  • Participate and assist your child in their continued treatment and probation, if applicable

Goal 2: Continued Sobriety

  • Assist your child in actively engaging in the methods identified in the post-graduation Success Plan to maintain their continued sobriety
  • Assist and encourage your child in attending continued substance abuse services as recommended
  • Attend and participate in your child's substance abuse services as scheduled
  • Encourage and assist your child in attending 12-Step, SMART Recovery or other community-based support group
  • Encourage your child to maintain regular contact with their identified sponsor or support network
  • Review your child's relapse prevention plan on a regular basis and assist in updating the plan as needed
  • Be mindful our your child's identified triggers and assist him/her in practicing the identified coping skills
  • Assist your child in keeping track of their days clean
  • Praise your child for their efforts and accomplishments on a regular basis
  • Encourage your child to avoid negative activities, drug-associated friends, locations, and continued practicing of law-abiding behaviors.

Goal 3: Continued Drug Court Support

  • Actively participate and encourage your child to engage in the methods identified in the post-graduation Success Plan to maintain continued Drug Court support and involvement
  • Encourage and support your child in becoming a Drug Court Volunteer (e.g., guest speaking, mediating groups, job shadowing, etc.)
  • Attend Drug Court Hearings with your child as recommended
  • Encourage your child in becoming a SEABHS Volunteer
  • Participate and attend Drug Court events, special groups or activities
  • Encourage your child in their involvement and attendance in Drug Court events, special groups or activities
  • Maintain regular contact with the Program Coordinator and the Drug Court Team Members
  • Encourage your child to maintain regular contact with the Program Coordinator and Drug Court Team members
