The Community Health Needs Assessment (CHNA) is a survey conducted every three to five years to help identify priority health needs and concerns in our community. This year Cochise County Health & Social Services (CCHSS) has partnered with LeCroy & Milligan Associates, Inc. to conduct the survey, blending resources and expertise to produce comprehensive, data-driven results. In addition, this collaboration will ensure that all voices are heard, with processes in place to reach the most vulnerable and underserved populations. Assessment of a community’s health is a core function of local health departments.
Cochise County residents should expect to receive a postcard in their mail and PO boxes as soon as this weekend prompting them to scan a QR code to reach the survey or to call 520-488-0167 to request a paper copy. Upon completion of the survey the first 1,500 participants will have the option to receive a $15.00 gift card. Cochise County Health & Social Services encourages the community to take this survey as soon as possible!
In addition to the survey, CCHSS and LeCroy & Milligan Associates, Inc. will be working with communities to ensure all voices are heard through community conversations and focus groups with various populations.
Once the report is complete, the community health assessment will serve as the baseline of community health data to be used by local hospitals, health centers, other health organizations and residents to support decision making, grant proposals, and health improvement programs in Cochise County. What will follow is the development of a Community Health Improvement Plan to address the health issues of concern identified through the CHNA survey.
